News: Medical Home Pilot Project and Learning Series Webinar


On Wednesday, November 17, 2010, TNAAP launched the Medical Home Pilot Project and Learning Series with an hour-long introductory webinar. Michael Warren, MD, Director of the Governor’s Office of Children’s Care Coordination and Deanna Bell, MD, Medical Director of Mercy Children’s Clinic in Franklin, TN were the presenters. TNAAP is now in the process of selecting 5 pediatric practices from across the state to participate as pilot sites. These 5 practices will receive support and resources from TNAAP to implement aspects of the Medical Home they are most interested in adopting. Selections will be completed in January, 2011.

The one hour webinar included:

  1. A review of the various Medical Home models and definitions.
  2. A discussion of the evidence based value of medical home implementation.
  3. An overview of one practice that has spent the last 2 years in the implementation process and their successes and lessons learned (including financial impact to the practice).
  4. A description of the TNAAP pilot program and the services that will be available to 5 pilot sites.
  5. A closing session of Questions and Answers.
